Nursing services

Jazai’s Nursing Services are designed to support individuals with complex health conditions or medical needs, ensuring they receive the medical care and monitoring they require while maintaining their independence.

Key aspects of our nursing support include:

  • Preauthorization Requirement: Before receiving nursing support services, participants must obtain preauthorization from the DDA, which is documented in their Person-Centered Plan (PCP).
  • Staff Oversight: Our services include ongoing supervision and training for staff, ensuring they are equipped with the knowledge to support participants effectively.
  • Registered Nurse Consultation: A licensed Registered Nurse (RN) in the State of Maryland is assigned to offer specialized Nursing Consultation, assessing and addressing the health needs of the participant.
  • Health Case Management: We provide Health Case Management services, coordinating care and ensuring all medical needs are met.
  • Delegation Services: We offer Delegation services, where nursing tasks are appropriately assigned to qualified staff based on each participant’s assessed needs.
  • Medication Management: Administering medications and monitoring for side effects or changes in health status.
  • Health Monitoring: Regular health assessments to track vital signs, weight, and any ongoing health conditions.
  • Chronic Disease Management: Providing support for individuals with chronic illnesses to manage their conditions effectively.
  • Coordination with Physicians: Collaborating with doctors and other medical professionals to create a comprehensive care plan.
